Notice Description

The Commissioner is looking to re-procure primary medical care services that are currently delivered through a time limited GMS contract. The current contract will expire at midnight on the 31st December 2021. The contract to be awarded will commence 1st January 2022 at one second past midnight for the initial term of 5 years, with the option to extend for a period up to further 5 years. The Awarding Authority will issue an APMS contract for this opportunity. The contract value will be based on the nationally agreed GMS global sum price applied to the practices' weighted list size revised on a quarterly basis. The GMS global sum price for 2020/2021 is GBP 93.46. The successful Provider will be expected to: * provide primary care services which are safe, effective, accessible, convenient and responsive; * design services around the needs of patients and carers, ensuring they are offered more choice and are empowered to make decisions in their treatment and care; * reduce unplanned admissions to hospital by delivering proactive and preventative care and enabling patients to access urgent care where appropriate; * have processes in place to engage effectively with patients, stakeholders and local communities and to evaluate feedback, complaints and compliments with a view to the continuous improvement of patient experience and satisfaction; * participate in national quality and outcomes framework (QOF); * be capable of delivery of Locally Commissioned and Enhanced Services, remunerated in line with the relevant Directions, as specified by the commissioner on an annual, or alternative frequency, basis; * promote healthy living with the aim of tackling the underlying causes of ill health and addressing health inequalities; * work in a network with other practices across the local area and participate in the Network Contract Directed Enhanced Service.
