Notice Description

Stockport Metropolitan Borough Council requires on-going Occupational Therapy Support assessments for a period of 4 years up until January 2027 which will need to be completed in accordance to timescales set out within each call to the market. 1 The Service will provide and carry out: a) occupational therapy assessments to identify service user's needs under The Care Act 2014, regarding the need for specialist equipment and major adaptations to promote wellbeing, safety and independence. b) arrange for the provision of any equipment and adaptations identified, to meet assessed need, and in accordance with Stockport Council's assessment guidelines and the DFG best practice guidance. c) moving and handling assessments for non-commissioned care. d) an efficient administrative support and information contact point for all relevant parties. e) accurate and clear documentation completed initially on Liquid Logic (Stockport's electronic client information system) following agreed standards and procedures. f) Liaison with DFG provider and contractors and retailers as appropriate g) Hold a case load of people waiting for adaptations or equipment, carry out regular reviews and end review to ensure the adaptation or equipment has met the persons needs.
