Notice Description

Camden Clinical Commissioning Group provision of Community Chronic Kidney disease (CKD) and hypertension services are currently provided by Royal Free London Hospital NHS Foundation Trust (RFL) in two separate contracts. During the recent service review, it was envisaged that an integrated CKD and hypertension service will support and align future long-term conditions for patients in advance of further development of the service delivery model. The proposal is for delivering an integrated CKD and hypertension service in Camden with RFL for a period of 21 months from 1st July 2018- 31st March 2020. The value of this 21 month is PS323,163. There is a possible 12 month contract extension to 31/03/2021, the value of the extension is PS184,665. RFL currently delivers virtual triage, nursing clinics, care planning, risk management, training and education, GPs engagement and community care for CKD and hypertension patients. Current contracts for these two services are to terminate on 30 June 2018. Camden CCG seeks to ensure that all future long-term condition service provision is provided within the borough of Camden wherever possible. Camden CCG can benefit from the efficiency gained by the integration of these services, which has the potential to benefit patients with co-morbidities. The integrated service will focus on service integration, improved patient journeys, achievement of improved clinical outcomes and value for money . The integrated service aims provide a seamless and consistent alternative service from 1 July 2018 and an alternate services provision would pose a serious challenge for service provision and may not be feasible for an alternative provider. Camden CCG will gain consistency, quality and better outcomes from RFL within this new arrangement. The long term plan for Camden CCG is to provide an integrated LTC service for the local population in the future and opportunities will be available to express an interest in developing this wider integrated service later in 2018.
