Enhanced Primary Care Services on behalf of NHS Harrow CCG
Notice Description
Provision of Enhanced Primary Care services for patients within Practices and Primary Care networks in Harrow. The services will support improvements in health and wellbeing, enable patients to access their care closer to home and will be flexible and adaptable, collaborating with other services where required for a seamless patient journey. These services will ensure parity and equity in service provision, through a requirement to meet 100% population coverage.
