Virtual Ward and Clinical Outpatient Services
Notice Description
The overall aim of the Virtual Ward and Clinical Outpatients Services Framework Agreement is to provide responsive and time limited support to individuals who are medically fit to receive treatment outside of an Acute hospital by providing defined clinical services and/or advice/information/signposting to inform a care pathway which enables patients to safely resettle at home and promote independent and choice. The service provider will facilitate appropriate interventions and provide pharmacy treatments at home or at outpatient clinics close to home. This will promote independence and reduce the likelihood of hospital readmission or long term care. The interventions will be prompt and provide access to services which enable timely discharge or admission avoidance.

Out of Hospital Injectable Services - Mobile Treatment UnitDefinition of Activity/ Services: Administration of hospital managed injectable therapies in a mobile treatment unit as part of long-term outpatient therapy, short-term supported discharge model or admission avoidance model.
Hospital at Home Virtual WardDefinition of Activity/ Services: Provision of a holistic personalised care package encompassing multiple healthcare disciplines to meet patient's needs as part of long-term outpatient treatment, short-term supported discharge or admission avoidance.
Clinical Support Services for OutpatientsDefinition of Activity/ Services: Personalised support service augmenting NHS clinic outpatient care to improve medicines adherence, patient activation / experience and clinical outcomes. To facilitate delivery of patient pathway from initial presentation to information, advice or treatment whilst avoiding any further delays to patients. To provide the relevant clinical or non-clinical staff, where appropriate, to perform clinical and support activities including administration, diagnostics and further therapies. Work closely with Participating Authorities to foster a positive working relationship with the goal being to maximise the effectiveness of appropriate evidence-based care.
