NHS Kirklees CCG (KCCG) is seeking to commission a provider for a community based anticoagulation service, to deliver a comprehensive service across either geographically dispersed locations or from a single site, including a home INR testing service for those unable to attend a clinic without NHS funded patient transport. The provider will deliver all elements associated with an anticoagulation service, including initiation, drug selection, dosing, dose adjustment and laboratory testing as well as undertaking monitoring and providing the patient-facing elements of the service. Patients will be referred by GPs or secondary care following diagnosis with a relevant condition. The service will be required to manage anticoagulation for all patients with conditions amenable to anticoagulation. Process Based on the above KCCG is keen to receive expressions of interest from suitably qualified potential bidders. Your response should include: 1. Overview of your organisation(s) and experience in providing an Anticoagulation Service in accordance with NICE Guidance 2. That your interest is based on a clear understanding of the requirements set out in this notice. 3. Confirmation of your capacity and capability to be able to mobilise the service to commence in April 2022. 4. Confirmation that you will be able work with the existing providers to ensure a seamless service from the start of the service. 5. Proposed service model, providing a timeline that confirms a. Level of capacity b. Cost per patient 6. Proposed service model that links to the requirements set out in this notice. An Expression of Interest, including the response to the above, is required by no later than the 10 May 2021.
