FRAMEWORK FOR THE PROVISION OF CARE PACKAGES FOR CHILDREN'S CONTINUING CARE
Notice Description
The Service provision will require Individual, bespoke Community Packages of Care for Children and Young People aged 0 - 18 years with Continuing Care Needs to enable Children and Young People to be cared for in their home. Care packages will encompass a broad spectrum of requirements, ranging from non-complex to more challenging complex cases including for example: - Care packages involving care plans developed by a qualified RSCN/RN Children's Nurse where there are low-level decision-making requirements. Care packages would have clinical oversight from the RSCN/RN Children's Nurse. - Care packages involving a more enhanced degree of decision making and responsiveness beyond following a plan of care provided by an RSCN/RN Children's Nurse, - Care packages requiring qualified nursing support (following risk assessment undertaken by the clinical team and the Commissioner's Complex and Continuing care team.
