Community Life Choices for Leicestershire
Notice Description
The Council is seeking a number of organisations to provide CLC services across Leicestershire to Adults aged 18 or over who are eligible for CLC Services and/or to receive a managed Personal Assistant service (PA's) following the completion of a Care and Support or Carers Assessment by LCC. The Service must be able to cover the needs in the Care/Support plan and note that this may include weekend and evening working hours. The primary purpose of the CLC Service is to enable people to remain in their own homes, living as independently as possible to achieve and maintain their potential in relation to physical, intellectual, emotional and social capacity. The provision of support will also be designed to help maintain the health and wellbeing of Carers. Service Providers should provide a level of support to assist people to build on their current strengths and develop their abilities to look after themselves without becoming dependent on support.
Lot Information
Lot 1
Service Providers will be offered packages based on: Lot 1 - Services for people with a Learning Disability and/or Autism o People in need of this service will have a range of needs, often related to multiple disability, health and/or behaviours where combination of specialist health and social care inputs are required Lot 2 - Physical and Sensory Disabilities o This can also include support needs which require joint intervention from health and social care specialists Lot 3 - Services for people with Mental Health needs o People with needs relating to complex and/or enduring mental ill health, dual diagnosis (e.g. physical, learning disability and substance misuse) Lot 4 - Specialist support for people living with Dementia o Dementia describes a set of symptoms that include loss of concentration and memory problems, mood and behaviour changes and problems with communication or reasoning. Lot 5 - Services for Older People aged 55 and over o This may include but is not limited to people with additional support needs and social care inputs Lot 6 - Service for people with Profound and Multiple Learning Disabilities (PMLD) o People will have severe learning disability and other disabilities that significantly affect their ability to communicate and be independent. Lot 7 - Managed Personal Assistant (PA) Service o Management of individual PA's with a view to the person managing their own PA in the future
