The care and support to be provided is to enable the following; an increase in the number of Individuals being able to continue to live independently their own homes, support Individuals to develop the skills and knowledge they need to live as independently as possible, a reduction in the number of Individuals entering into residential care or nursing home care, a reduction in the number of people who are supported outside of the Darlington area, a reduction in the number of emergency admission to hospital and admissions into Assessment and Treatment Services, the facilitation of timely and safe discharge from hospital and to support unpaid family members and carers to continue in their caring role
