Summary of the contracting process

The procurement process involves a modification notice for the General Medical Services (GMS) Contract for Blakeney Surgery, now updated to include services for patients previously registered with Coleford Medical Practice. The buying organisation involved is the NHS Gloucestershire Integrated Care Board, located in Gloucester, UK. This procurement process, specifically at the award update stage, falls under the health services category and is classified under a services procurement category. The contract, originally signed on 1 August 2025, maintains a duration of operation in perpetuity, indicating its ongoing nature without a defined end date. The rationale for this modification was due to unforeseen circumstances that necessitated changes to maintain stability within the Primary Care Network in Gloucestershire.

The tender offers significant opportunities for business growth particularly for healthcare service providers capable of managing large patient lists and delivering consistent primary care services. Given the perpetual nature of this contract, businesses in the healthcare service sector, especially those with experience in primary care provisioning, would benefit from considering this opportunity. With an annual value indicated at approximately £2,029,189, it offers considerable financial potential for a well-suited provider like GDoc Ltd, which is already integrated in the Gloucestershire healthcare landscape and capable of accommodating the expanded service delivery requirements. The tender also showcases the need for reliable providers who can ensure continuity of care amid changing service delivery demands.

Notice Title

GMS Contract for Blakeney Surgery

Notice Description

The novation of a General Medical Services contract to G Doc Ltd: NHS Gloucestershire CCG held a General Medical Services (GMS) contract ("the Contract") with the Partners of Blakeney Surgery ("the Partnership") for the provision of primary care services from Blakeney Surgery. The Contract is subject to the provisions of Chapter 3, Section 7 of the Public Contracts Regulations 2015 ("the PCR") (the Light Regime). NHS Gloucestershire CCG has relied on Regulation 72(1)(d) of the PCR to effect a permitted variation to the Contract given that G Doc Ltd has succeeded into the position of the Partnership, taking over its business operations. The Contract was novated to G Doc Ltd on 1 June 2022 on the same terms.

Lot Information

Lot 1

The modified contract will cover the provision of primary care services to patients registered with Blakeney Surgery and also those registered with Coleford Medical Practice. The Blakeney GMS Contract is a GMS Contract that is subject to the requirements of the National Health Service (General Medical Services Contracts) Regulations 2015 (as amended).

Procurement Information

The Partnership was made up of two GPs who wished to withdraw from running the practice. GDoc Ltd was chosen by the Partnership to take over the Partnership's business operations. GDoc Ltd has replaced the Partnership as a consequence of a universal succession into the position of the Partnership which will now be wound down. The CCG therefore considers that the novation is permitted under Regulation 72(1)(d) of the PCR insofar as this Regulation is relevant to contracts that fall within Chapter 3, Section 7 of the PCR (contracts for social and other specific services). GDoc Ltd is a Gloucestershire-based cooperative of general practitioners which was willing and able to take over the Blakeney Surgery practice. It meets the requirements of the GMS Regulations and is a current contract holder in Gloucestershire for primary care services. The benefits of being able to novate the contract to a known entity in terms of assured continuity of care given the difficulties with workforce recruitment and retention in this rural area are significant. The novation of the Contract to GDoc Ltd ensures the ongoing stability of primary care services in Blakeney and enables these crucial services to continue without any break in service following the decision of the Partnership to withdraw. One of the GPs that comprised the Partnership continues to be employed within the practice and fully supports and is committed to the population in Blakeney. The CCG is satisfied that GDoc Ltd is the only provider capable of achieving these objectives at this stage.

"description": "The Blakeney GMS Contract has been modified to include the provision of primary care services previously provided by Coleford Medical Practice with effect from [date]. As a result of this modification, the practice boundary has changed and the patient list has increased such that the contract now serves 16,880 patients (an increase from 3,283 patients). The length of the contract will remain the same as it will continue to operate as a contract held in perpetuity. There is no lifetime value of the Blakeney GMS Contract because it is a contract held in perpetuity and the annual contract values change year on year. The value of the contract is calculated in line with the General Medical Services Statement of Financial Entitlements Directions (as amended from time to time). As an indication of values based on 2024/25 figures, the annual global sum payment for Blakeney Surgery was PS399,765 and the inclusion of the additional Coleford patients would have increased the value of the Blakeney GMS Contract by PS1,629,424 to a total annual value of PS2,029,189.",
"rationale": "Need for modification brought about by circumstances which a diligent contracting authority/entity could not foresee: The ICB considers that the modification is permitted pursuant to the Health Care Services (Provider Selection Regime) Regulations 2023 under regulation 13(1)(c) i.e. where the modification is made in response to external factors beyond the control of the ICB and the provider. The existing Coleford GP Partners all wish to step down as contract holders. The provider holding the Blakeney GMS Contract (G DOC Limited) operates in the same Primary Care Network and is in a position to provide the services in the Coleford area under its existing contract. The existing Coleford GP Partners stepping down is an external factor that is beyond the control of both the ICB and G DOC Limited (the provider). If this were to happen and the GP Partners were not replaced (either by way of an individual, a partnership or a company), Coleford Medical Practice would not be able to operate as there needs to be an entity that is responsible for the delivery of the services. It is therefore necessary to modify the Blakeney GMS Contract in response due to the integrated nature of primary care services: * This enables continuation on a GMS contract model with dispensing rights (there being challenges procuring an APMS model in this geographical area) * The cessation of Coleford Medical Practice would lead to patients having to register at other GP practices (thereby causing disruption to their operations) and/or patients in both the Blakeney and Coleford area not having access to the care required due to the reduction in access to GP services locally. As such, if Coleford Medical Practice were to fail it would destabilise and disrupt the whole Primary Care Network. * Blakeney Surgery is within the same Primary Care Network as Coleford Medical Practice and would therefore be impacted by any disruption arising from the failure of that practice. This impact could potentially be fairly significant given the size of Blakeney Surgery (c. 3,000 patients) and the size of Coleford Medical Practice (c. 13,000 patients). Blakeney Surgery therefore would not have the capacity to absorb the patients currently listed at Coleford Medical Practice. * As Blakeney Surgery is fairly small, there is a reasonable chance that the ICB would need to consider options in the near future to ensure its ongoing viability. Modifying the Blakeney GMS Contract to incorporate the Coleford Medical Practice premises will mitigate against the risk of these viability issues. It also aligns with the direction of travel from a national policy perspective in terms of creating more sustainable primary care delivery models. On balance, the ICB does not consider that the Blakeney GMS Contract will be rendered materially different in character as a result of the change as G DOC will be continuing to provide the same primary care services to patients."

"description": "This is a Provider Selection Regime (PSR) contract modification notice, under the Health Care Services (Provider Selection Regime) Regulations 2023. For the avoidance of doubt, the provisions of the Public Contracts Regulations 2015 do not apply to this modification. This contract has now been formally modified. The decision to modify the contract was taken by the Primary Care and Direct Commissioning Committee under delegated authority by NHS England. No conflicts of interest registered. COI is managed using the ICB COI process along with NHSE COI guidance and the PSR reg 21 and guidance."
