Flintshire County Council are posting this notice to ascertain the level of interest in the market to provide the service/s detailed below in advance of a full tender exercise being carried out. Lot 1
Title - Social Prescribing and Early Interventions - Wellbeing Officers. This is a new service funded by the Welsh Government's Regional Integration Fund (RIF) Programme as part of the Community Services Transformation Programme. This service will provide a small team of Wellbeing Officer roles across the community in Flintshire. The Wellbeing Officers will work with individual adults in the local community to provide short term support and to ensure that their health and wellbeing needs are being met. A preventative and early intervention approach will be adopted to provide coordination of practical support to individuals identified as being at higher risk of escalating needs. Including support for people following hospital discharge. The Wellbeing Officers team will be employed by the awarded provider. The team will be required to work collaboratively with multi-disciplinary teams, including health and social care professionals in Betsi Cadwaladr University Health Board and Flintshire County Council. The team will also be required to develop strong collaborative links with primary care providers, including GP and health centres, pharmacy services as well as community groups and organisations. Work allocation will be directed from a Social Services team using established referral processes. The Wellbeing Officers will build strong working relationships with the Flintshire Local Voluntary Social Prescribers team based within the Single Point of Access (SPoA) base in Mold. This Social Prescribing and Third sector Coordination Team receive a high volume of referrals from a broad age range and work with third sector service providers and support organisations to find person centred solutions for individuals. The Wellbeing Officers will have a wide remit, providing day-to-day support and advocacy, developing short term practical approaches to ensure that people are able to maintain their health and wellbeing in their own home. Enabling people to access prescribed services and support, attend medical or social appointments. The Council would like to invite Providers with demonstrable experience of delivery of relevant support services to further discuss the opportunity to be a delivery partner within Locality Multidisciplinary Teams Partners for this contract will demonstrate a clear vision for the services, with a focus on individual outcomes using person centred plans and prevention and early intervention models of care. Partners would share the vision to maintain social care values, increase integration and interaction between services and local communities by utilising commercial partnerships, social commitments and volunteer participatory agreements. This is an exciting opportunity for a provider with expertise and a proven track record in providing similar services, to take ownership and transform a well-established and valued service, with skilled and experienced staff. The commissioned provider will continue to deliver high quality support and development for people, offering value for money and delivering outcomes to meet the needs of the local population.
