Notice Description
NHS Arden and Greater East Midlands Commissioning Support Unit (AGCSU), on behalf of NHS England Highly Specialised Services (referred to as the Commissioner), is undertaking a market engagement exercise to understand the current and future capacity and capability of providers and potential providers within this highly specialised area. All interested parties, including incumbent providers, are encouraged to register their interest and participate in the market engagement questionnaire which will provide feedback to commissioners. Please note that this is not a call for competition although the commissioner would encourage all potential providers to respond to this market engagement exercise. The outcome of this market engagement will help inform commissioners of the current and future capacity and capability of the market and will be considered to inform the commissioning approach.
Lot Information
i) Lot 1 - North comprising the NHS England Regions: the North East and Yorkshire; the North West
This market engagement exercise is to re-commission a service for the provision of the Total pancreatectomy with islet autotransplantation (TPIAT) service from April 2023. NHS England is looking to re-commission TPIAT services for adult patients with hereditary chronic pancreatitis and who are at high risk of developing pancreatic cancer and for patients with acquired intractable chronic pancreatitis who meet specific clinical criteria The primary goal of surgery is to remove the cause of the symptoms i.e. the pancreas (total pancreatectomy) with an aim to control pain resistant to other therapies; islet auto transplantation (a procedure where the patient's own islets are isolated and infused into their liver) is intended to prevent or lessen the very brittle (hard to control) diabetes mellitus which is an inevitable result of total pancreatectomy. We would like to receive responses from incumbent and potential provider(s) who are able to meet all of the following key criteria: a) Expertise in the assessment and management of patients with chronic pancreatitis; b) Access to a specialist pain management service; c) Experience of pancreatic surgery and expertise in the management of patients who have undergone total pancreatectomy; d) Experience of islet transplantation including an on-site islet isolation laboratory licenced by the Human Tissue Authority conforming to Good Manufacturing Practice; e) Having in place a specialist team with expertise in the management of complex Type 1 diabetes mellitus, including the full range of therapeutic options (including insulin pumps and psychological assessment) and expertise; * (The above criteria may be evidenced for example by, number and range of patients on caseload, multi-disciplinary team experienced in the care of patients with chronic pancreatitis, published research papers) f) Having in place information governance processes that permit the collection of the specified clinical outcome data; g) Experience of engaging positively with associated patient support groups. Incumbent providers are required to respond to this market engagement exercise along with any prospective providers who can meet the service requirements. NHS England is looking to re-commission TPIAT services for adult patients with hereditary chronic pancreatitis and who are at high risk of developing pancreatic cancer and for patients with acquired intractable chronic pancreatitis who meet specific clinical criteria The primary goal of surgery is to remove the cause of the symptoms i.e. the pancreas (total pancreatectomy) with an aim to control pain resistant to other therapies; islet auto transplantation (a procedure where the patient's own islets are isolated and infused into their liver) is intended to prevent or lessen the very brittle (hard to control) diabetes mellitus which is an inevitable result of total pancreatectomy. We would like to receive responses from incumbent and potential provider(s) who are able to meet all of the following key criteria: a) Expertise in the assessment and management of patients with chronic pancreatitis; b) Access to a specialist pain management service; c) Experience of pancreatic surgery and expertise in the management of patients who have undergone total pancreatectomy; d) Experience of islet transplantation including an on-site islet isolation laboratory licenced by the Human Tissue Authority conforming to Good Manufacturing Practice; e) Having in place a specialist team with expertise in the management of complex Type 1 diabetes mellitus, including the full range of therapeutic options (including insulin pumps and psychological assessment) and expertise; * (The above criteria may be evidenced for example by, number and range of patients on caseload, multi-disciplinary team experienced in the care of patients with chronic pancreatitis, published research papers) f) Having in place information governance processes that permit the collection of the specified clinical outcome data; g) Experience of engaging positively with associated patient support groups. The services will be provided in four centres (one per defined geographical catchment area) which provide a high volume Hepato-pancreatobiliary service with significant experience in managing severe chronic pancreatitis, carrying out pancreatectomy; undertaking islet cell transplantation and having in place an appropriate multi-disciplinary team. Incumbent providers are required to respond to this market engagement exercise along with any prospective providers who can meet the service requirements. The Lots will be as follows: i) Lot 1 - North comprising the NHS England Regions: the North East and Yorkshire; the North West, ii) Lot 2 - the Midlands comprising the NHS England Regions: the Midlands; the East of England iii) Lot 3 - London comprising the NHS England Region: London iv) Lot 4 - the South: comprising the South East and the South West The questionnaire is available through the EU Supply portal: https://uk.eu-supply.com/login.asp?B=agcsu The reference is 62207 and the closing date for submission of the completed questionnaire will be: 12pm, midday on Tuesday 20th December 2022.
