Derbyshire County Council, in conjunction with local partners the National Probation Service (NPS), Safer Derbyshire Community Safety Partnership, and the Office of the Police and Crime Commissioner, is seeking to commission a service provider to deliver a health trainer service to adult offenders on community orders or licences delivered by Derbyshire Probation Services for the areas covered by the Chesterfield office (map attached as appendix). A snapshot audit in October 2022 identified that the Chesterfield offender cohort consists of an approximate caseload of 862 clients, with around 700 in the community. There are around 213 HR cases in total of which 132 are in the community. Health Trainer services support adults who wish to make a change towards a healthier lifestyle but who are unlikely to make and maintain this without individual support. Probation Health Trainers are often ex-offenders, and all health trainers must have successfully completed the nationally accredited Health Trainer training and been assessed as competent to undertake the Health Trainer role. Health Trainers work with clients on a one-to-one basis to assess their health and lifestyle risks and support them in their decision to make and sustain behavioural change to a healthier lifestyle. The service will commence on 01 August 2023 with an end date of 31 July 2025. The contract may be extended up to a further two 12-month periods up to 31 July 2027, subject to funding and satisfactory performance.
