Notice Description
The service is geared towards delivering improved outcomes for people living with dementia, enabling them to regain their independence within their own home environment through input from personalised support to aid the service user to reach their enablement potential. The service is tendered under two lots:Lot 1: Occupational Therapy Led Support - Enabling individuals to regain independence through physical support and reablement. Lot 2: Cognitive Dementia Professional - Specialist support and cognitive input will be aimed at developing an awareness of the reasons behind the behavioral patterns and activities of the individual, which, when understood, will feed into the support tailored for each individual.The service will deliver a flexible, short-term (to a maximum of six weeks), promoting independence service, both from hospital discharge or step up from the community.
Lot Information
COV - Dementia Community Promoting Independence Service - Lot 1 - Occupational Therapy Led Service
Lot 1: Occupational Therapy Led Support Enabling individuals to regain independence through physical support and reablement. Physical support will be aimed at recovering, developing and maintaining the ability to carry out everyday tasks within their home or community.
Renewal: The contract will run for an initial 3 years with the option of a further 2 year extension.
COV - Dementia Community Promoting Independence Service - Lot 2: Cognitive Dementia ProfessionalSpecialist support and cognitive input will be aimed at developing an awareness of the reasons behind the behavioral patterns and activities of the individual, which, when understood, will feed into the support tailored for each individual.The service will deliver a flexible, short-term, promoting independence service. Service users receive support up to a maximum of six weeks. This may be on discharge from hospital to enable them to return home safely and to regain their independence, or as a step up from the community to those people who require additional support to maintain their independence.
Renewal: The contract will run for an initial 3 years with the option of a further 2 year extension.
