Notice Description

The NHS Integrated Care Board (ICB) based in Doncaster in partnership with Doncaster City Council are looking to enter a procurement process for to provide suitable and sustainable long term community solutions for four Doncaster patients who have been identified as requiring specialist community placements.

Lot Information

Lot 1

All four patients have a long history of inpatient admissions with the average length of stay being 7 years. Three patients present with autism and Emotionally Unstable Personality Disorder (EUPD) diagnoses and one with Autism/ADHD. Previous discharge and community placement arrangements have failed and learning from those experiences has led the council and ICB to propose a new approach.
To build a successful community home the provider must have confident support and care staff who are appropriately experienced, skilled, competent and resilient. Staff must be provided with support and supervision matching with their workload and relevant continuing professional development.
Managers and staff of the Service will have knowledge and expertise in relation to:
* People with learning disabilities and/or Autism
* Challenging Behaviour including self-injurious behaviour and disorders of eating
* Devising and delivering Positive Behavioural Support plans with a heavy emphasis on getting a person's life in balance as opposed to reactive behaviour management, where good practice identifies this is an appropriate approach using recognised PBS frameworks and evaluation tools
* Personality Disorder and Attachment Disorder; the effects of early life trauma
* Issues related to offending and the Criminal Justice System e.g. Police, Court Liaison, Probation
* People who misuse substances (though not their primary need)
* Trauma informed care
* The early identification of deterioration in mental health
* The prompt identification of physical health needs and how to respond to these
* Safeguarding, Advocacy, Equality, Diversity & Social Inclusion
* Quality monitoring to evidence outcomes and continuous improvement
Personal profiles are provided as part of this PIN along with a draft service specification.
The health and social care opinions are that all the patients require their own self-contained and specifically adapted accommodation with an intensive support package in or near Doncaster to reduce risk and increase successful step down to their community home. However, owing to the complex health and social care needs of the patients; individually commissioned in isolation care packages are also considered to be at high risk of placement breakdown owing to the potential for staff burn out. Commissioners are seeking to procure a new solution specifically to address the long-term housing and support needs of these four patients by providing a sustainable solution.

The proposal is for a dispersed model of accommodation with the same care provider supporting all four individuals.

    "description": "The accommodation will involve a cluster of four homes within a 10-15-minute drive of each other which will be owned or acquired by a compliant Registered Provider of Social Housing. It is envisaged a bid for NHS capital will be made by the Registered Provider to help with the purchase and adaptation costs of the accommodation. This will help to ensure the rents are affordable to housing benefit. <br/> <br/>At the same time Commissioners will procure a specialist care provider to deliver the support service to all four patients. It is hoped that by providing an economy of scale of four care packages, the care provider can ensure the necessary management support that will be required to effectively deliver for individuals with such complex presentations. The staff team can work across the four people with the aim of reducing the risk of burnout and helping to ensure continuity of support for the individuals. <br/><br/>The outcomes<br/><br/>Through this project the aim is to achieve<br/>* Support to move people into a community home as step down<br/>* individual bespoke housing and support arrangements, with the back-up of a shared support service. <br/>* access to a wider support team to allow care to be flexed to meet an individual's needs as they change<br/>* care that can be offered in the least restrictive way possible for each individual.<br/>* To create an opportunity for the designated care provider to facilitate robust relationships with community support teams. <br/>* a reduced risk of placement failure.<br/><br/>It is proposed the financial envelope will be up to PS8k to PS10k per week per individual where 2:1 24 hours staffing ratio is required; it is expected the provider works to reduce this staffing ratio. <br/>The purpose of this PIN is to identify any organisations interested in providing this Service to engage with NHS South Yorkshire Integrated Care Board (NHS SY ICB (Doncaster). The SY ICB would like potential providers to express an interest by responding via the portal. The SY ICB are not liable for any costs incurred by those expressing an interest in this PIN or, for any future tendering activity.<br/>Please register your interest through the Atamis portal by the 5th May 2023.<br/>The information you provide to SY ICB (Doncaster) will be to assess interest in the market.
