North Central London (NCL) Integrated Care Board (ICB) wishes to continue to commission services delivered in a primary care setting from individual GP practices and the Islington GP Federation. The locally commissioned services aim to improve outcomes in these clinical areas.

Lot 1
NCL ICB commissions a significant number of Locally Commissioned Services from General Practice. These services are provided in general practice but are not part of the General Medical Services contract. They are delivered by practices or through a GP Federation. NCL ICB wishes to continue to commission services delivered in a primary care setting from individual GP practices and the Islington GP Federation. The locally commissioned services aim to improve outcomes in these clinical areas. * Anti-coagulation * Autism * Cancer Care * Care Homes * Care for Homeless * Care for Asylum seekers * Frailty * High Risk Drugs * Last years of life * Long Term Conditions * Medicines Management * Primary Care Access/Development/Infrastructure * Phlebotomy * Sexual Health * SMI Healthchecks * Smoking cessation * Tele-dermatology/dermatology * Tongue Tie Service * Vaccinations The value of each contract will be determined upon practice sign up, achievement and weighted according to their patient list size. Contract will be for 3 years with option to extend by a further 2 years. Additional information: NCL ICB commissions a significant number of Locally Commissioned Services from General Practice. These services are provided in genera practice but are not part of the General Medical Services contract. They are delivered by practices or through a GP Federation. The nature of the services to be provided and the need to be provided across primary care within Islington means that only a list-based procurement could be undertaken. The value of each contract will be determined upon practice sign up, achievement and weighted according to their patient list size.
