BNSSG ICB has implemented a Discharge to Assess model following hospital admission, in collaboration with ICS partners. The overall model includes four pathways: Pathway 0 - 'simple' discharge to usual place of residence with no or low levels of ongoing support. Pathway 1 - discharge to usual place of residence with short term package of care to support rehabilitation and reablement (usually 10days), possibly leading to long term care support package most probably at home. Pathway 2 - intensive, bed-based rehabilitation (usually 21 days), requiring a community hospital or similar nursing home setting, possibly leading to long term care support package most probably at home.
