The vision of the model is to maximise independence, de-escalate care as soon as possible to prevent de-compensation and over-time this is expected to increase the number of people managing independently or with care support at home and reduce the numbers of people relying on bed-based care in the short term and long term. The Contract term will be 18 months initially, with the option to extend by any further period or periods up to a maximum of 12 months. The Contract is planned to commence on 1 October 2023. The total value of this procurement including extensions is PS22,007,017. This procurement is being carried out by NHS South, Central and West Commissioning Support Unit (SCW) on behalf of the Commissioners.
