Facilitating a range of education sessions and peer support for people living with Dementia and their carer(s) in all areas of Derbyshire.
Options: Contract contains the option to increase the capacity of the service to deliver additional volume for additional service demand up to the value of PS500,000 over the lifetime of the contract, subject to a range of conditions.
