Notice Description
The overarching aim of this service is to support people to live independently in their own homes as long as possible. This will be achieved through the following objectives: * Deliver Wirral Independence Service including the Telecare Monitoring and Response service and Falls Prevention Service * Provision of a Community Equipment Service (CES) for adults and children within the geographical boundary of Wirral Borough Council including those who live out of Borough and are registered with a Wirral GP. The service includes. * Pressure care, * Continuing Health Care (CHC), * Adult Visual Impairment & Hearing Impairment equipment, * Telecare and any future innovations (equipment, installation, maintenance, monitoring, decontamination and repair and recycling). * Mobility aids, * Moving and Handling, * Bariatric, * Schools Specialist Equipment, * Paediatrics, * Nebulizers * Toileting * Specialist non catalogue items * Minor Adaptations * Carers Emergency Response Cards Incorporating: * an online IT requisition. * monitoring performance dashboard and reporting system, * Community based Retail Outlet/ Demonstration facility (Centre for independent living) * online self-help/assessment system and to include purchase and recycling opportunity * suitably qualified, knowledgeable, and experienced staff to support those funding their own care and to provide professional assessment and oversight within the service.
