A Discharge To Assess Bed Primary Care Cover Service in Plymouth to provide safe, effective and agile Primary Medical care to patients who are in Bedded Intermediate Care. The care provided will be of equivalent scope as would be provided by a patient's own GP practice and indicatively include: * the personalised, Primary Medical care and continuing nature of the care of individuals * the physical, psychological and social aspects of diagnosis * the early diagnosis, initial decisions and continuing management of problems * acute, chronic and terminal types of illness * treatment, prevention and education for promoting health This procurement is being carried out by NHS South, Central and West Commissioning Support Unit (SCW) on behalf of the Commissioners Additional information: The Contract will be for an initial term of 3 years, with an option to extend for any period up to a further 12 months, at the sole discretion of the Commissioner. Services are scheduled to commence on 1st April 2024. The services are healthcare services falling within Schedule 3 to the Public Contracts Regulations 2015 ("the Regulations") which are not subject to the full regime of the Regulations, but are instead governed by the "Light Touch Regime" contained within Chapter 3, Section 7 of the Regulations (Regulations 74 to 77).
