Lot Information
Lot 1

There will be an expectation that anybody admitted to a D2A bed within this model will have access to the therapy service which will: Provide a dedicated therapy service consisting of HCPC registered physiotherapists, occupational therapists and therapy assistants who have experience of working with older adults, some of whom will have complex needs and multiple co-morbidities. Work with people using an asset-based approach, building on their strengths and agreeing reablement goals for individuals. People's progress will be closely monitored, and goals will be regularly reviewed throughout the reablement period of up to 6 weeks.Work in line with the Suffolk D2A principles where our goal is to maximise the chances of people returning home to live as independently as possible. Participate in and contribute to an MDT approach that seeks to optimise people's outcomes and provide high quality care. This model provides a minimum of 30 beds. Should bed capacity or occupancy be at a lower level for a prolonged period, discussions would take place on how to utilise any spare capacity in the therapy service, this could include flexing capacity of the therapy service to other local sites.
