The Specialist Assessment of the Frail Elderly (SAFE) services delivers proactive care for people at risk of, or living with, frailty in North Lincolnshire. The GP-led specialist service undertakes holistic Frailty Assessments for a proactively identified patient cohort and develops person-centred care and support plans, including advanced care planning where appropriate, for those individuals. The service is delivered in the patient's own home or in a clinic setting, based on individual's need. The service provides both 'routine' and 'rapid access' appointments. Rapid access appointments are offered for those patients where there is a high risk of hospital admission within the next 2 weeks, or where a patient has been discharged from Same Day Emergency Care (SDEC) or acute Frailty service and is at risk of readmission.
