Lot 1

Provision of Healthy Weight Services delivering an evidence-based multi-component Tier two offer to support both the prevention and management of excess weight in children, young people and their families (age 5-16, up to 25 with SEND), those who are care experienced, adults and pregnant women. The model is a Tier 2 service, with Tier 1 universal messaging and health promotion to help people change behaviour and self-manage their lifestyle behaviours. This is new service with a new service provider and the lifetime value is PS2,460,960. The contract is expected to start on 01/11/2024 for a period of three years with the option to extend by a further two years (+ one year + one year). This notice is an intention to award a contract under the competitive process of the Health Care Services (Provider Selection Regime) Regulations 2023

Renewal: 3 years initial term with 2 x 1 year extension option
