Allowing people to settle and return to their optimal baseline of health prior to Adult Social Care, or other health professionals' undertaking long term care need assessments. * To improve the individual's physical and mental health and wellbeing, to a point where they can be safely discharged home, with or without a package of care, or into a long-term care placement. * To help adults in Medway to live independently for as long as possible.",
