Cardiff Council is committed to providing a model of Individualised strengths based and co-produced services, which will promote and maintain the independence and inclusion of Individuals who are experiencing mental health illness or borderline learning disability and who may have a range of other social, physical and substance misuse related difficulties. The main principles of supported living are to increase each person's individual independence and skills through the provision of tenancy-based accommodation and care and support for those experiencing mental ill health and a range of other social, learning, physical and substance misuse related difficulties. Supported living recognises that people have the right to make choices and be in control of their lives. Supporting people to choose where they live, with whom, what job they may have, what they do for enjoyment, what relationships they have, are all enabling factors creating a real sense of citizenship and social inclusion. This supported living model is a Core and Cluster model where people have individual tenancies in blocks or clusters of flats with each person receiving an individual care and support package. The service procured under this contract will enable Individuals to live as independently as possible. All people accessing the service will be assessed in accordance with the Social Services and Well-being (Wales) Act 2014 and have identified individual outcomes. The purpose of supported living is to enable adults, who are experiencing mental ill health and a range of other social, learning, physical and substance misuse related difficulties, to achieve their full potential, which includes improved mental and physical health and emotional, psychological and social needs being met. The Provider will achieve this by the sensitive recognition of the potential in each Individual and by understanding that it may change over time. The service will provide long-term accommodation where Individuals will have appropriate legal tenancies, domiciliary care and support.
