                    "rationale": "Need for modification brought about by circumstances which a diligent contracting authority/entity could not foresee: * Following a paper based early market engagement exercise and a stakeholder engagement event, there is interest from the incumbent provider to deliver this contract under the existing GMS contract as a branch site to Birchwood Surgery, which would be the main site for this contract * The current CQC rating is Good across all domains * The practice has an active PPG and conducts regular patient surveys which are published on the practice website. The last one was undertaken in 2020 and the results were between 4-5 stars out of 5 stars across 5 questions. The PPG is planning to run another survey this year * In the latest GP Patient Survey, the practice was above national and ICB average for 'overall patient experience' and for 'experience of making an appointment' * This option would be in the best interests of the patients and would be the most cost effective. * Creation of a branch is less disruption to the patients and faster mobilisation to ensure continuity of care For clarity and transparency purposes, all values mentioned within this notice are annual values. Lifetime value is unknown and subject to patient list fluctuations and Global Sum increases as per the General Medical Services Contract."
