The contract modification will be added to main Acute contract with an annual break clause and will be reviewed annual basis. The specialist obesity service may be subject to change due to elements such as, ITU arrangements and wider NHSE commissioning frameworks relating to weight related medications. National frameworks have indicated a 3-year review point to re-assess delivery options which will include e.g. access to Tirzepatide.",

                    "rationale": "Need for additional works, services or supplies by the original contractor/concessionaire: The NICE \"Obesity: Identification, Assessment and Management\" guideline (CG189), highlights the importance of and recommends moving beyond a tiered approach to obesity care. In line with this, SNEE's new integrated weight management and complex obesity disease pathway ensures that patients receive the appropriate support from the right level of service, according to their needs. The provider will offer an integrated service model, collaborating with a range of weight management service providers to deliver both pre- and post-complex obesity care, and leads assessment, clinical prioritisation and triage for the wider weight management pathway. This model also ensures an integrated pathway across medical and surgical interventions. There remains a recognised incidence of serious postoperative complications. Consequently, there is a requirement for critical-care resources for the occasional post-bariatric surgery patient with direct access to an on-site Intensive Treatment Unit (ITU)."
