Telford & Wrekin Council are looking to receive applications from Providers to join a multi-provider Framework to deliver specialist care services for Adults with Learning Disabilities, Autism, Mental Health and Acquired Brain Injury needs, in suitable locations. We want to work with established, experienced providers who deliver a curious, compassionate and solution focused approach central to delivering a person-centred service. The services we intend to commission are intended for adults, but it is expected that providers, where appropriate have the correct registration to support young adults during transition and working in line with the Preparing for Adulthood (PfA) principles and support delivery of outcomes around employment and training, friendships, relationships and communities, independent living and good health. The services will be split into 4 Lots covering increasing levels of need and complexity.

Lot Information

Living Well

Community Led support (including that delivered by partners in the Voluntary and Community Sector) Living Well forms the basis of all Lots. The principles and ethos of Living Well apply whether someone has complex needs or lower-level needs. We want people to be part of the community in which they live. This is not typically regulated provision and is provided to people with: * Learning disabilities * Autism * Mental Health * Acquired Brain Injury The value of the Framework will be shared across all Lots.

Regulated Community Support

For people with moderate level needs. This includes regulated provision in relation to: * Learning disabilities * Autism * Mental health * Acquired Brain Injury The value of the Framework will be shared across all Lots.

Specialist Complex Community Support

Support for people with high level support needs. This includes provision for people with: * Learning disabilities * Autism * Mental health * Acquired Brain Injury * Overnight support In addition to this, people supported by Lot 3 services may have complex behaviours and or health needs and / or dual diagnosis. The value of the Framework will be shared across all Lots.

Residential Services

For people requiring specialist Residential Services. * Learning disabilities * Autism * Mental health * Acquired Brain Injury The value of the Framework will be shared across all Lots.
