Notice Description

1) This notice is placed as a Prior Information Notice (PIN) to inform the market of NHS Devon Integrated Care Boards (ICBs) intention to recommission Primary Medical Services in Cranbrook, Devon. The Commissioner would like to understand the level of provider interest in delivering these services. 2) NHS Devon Integrated (the Commissioner) is seeking to secure a Provider who has the capability and capacity to deliver general practitioner services, in line with the specified requirements. 3) The geography covered is: Cranbrook is a new town being developed in East Devon. It is located 9 kilometres (5.6 miles) east; north-east of Exeter city centre, just north-west of the village of Rockbeare. 4) The contract duration will be confirmed ahead of the formal invitation to tender (ITT) being issued. At present the anticipated start date is 01 April 2026 as a 2 year APMS (with potential to extend for a further year) and where applicable and dependent on the Commissioner having assessed satisfactory delivery of APMS and that a transfer of service to a GMS contract would give needed resilience to the delivery of Primary Medical Services for the population, the Commissioner may agree to such a transfer of contract at a date to be set by, strictly, mutual agreement. 5) The town of Cranbrook will continue to expand over the next two years. There has been a steady increase in the practice population of c500 patients each year. 6) TUPE may apply 7) 2024 Annual Contract Value PS432,767 8) The contractor will also receive funding in relation to its participation in QOF, Directed Enhanced Services and Local Enhanced and Locally Commissioned Services. 9) Interested parties will need to register their interest via the Atamis portal (link provided below) by no later than 12:00 Friday 13th June 2025. This exercise is being carried out by NHS South, Central and West Commissioning Support Unit (SCW) on behalf of the Commissioner.

Cranbrook is a new town being developed in East Devon. It is located 9 kilometres (5.6 miles) east; north-east of Exeter city centre, just north-west of the village of Rockbeare. The contractor will identity design and deliver primary care services to the population covered by Cranbrook Medical Practice. The aim is to ensure that primary medical services are designed and delivered are reflective of a whole system framework for the delivery of high quality health and wellbeing services as an integrated approach ensuring cross working with different health and social care providers across the Eastern Locality and Eastern Local Care Partnership of NHS Devon Integrated Care Board. To achieve this aim, the Contractor must ensure: * Flexibility in the design and delivery of primary care services. To have patient-centred and modern design and delivery of primary medical services such that services are accessible, patient experience is good and patients do not, from their experience, see reason to seek re-registration with another practice. This will include (though not exclusively): o Continuity of Care and Long-Term Conditions (routine) - services will be designed and delivered to accommodate a rising, ageing population and the associated increased need for continuity, balancing against those patients requiring less continuity but still requiring timely access to care. o On the Day care - services will be designed to accommodate those who are generally in good health, digitally aware - happy to have online responses and continuity is less of a priority. The provided will need to facilitate, and support, scaled-up on the day access for General Practice where needed. o Preventative care - the Contractor will design and support innovative preventative care for its population. * Multi-disciplinary working across adult and children social care; community mental health teams; community nursing teams and other health and social care teams * Positive collaboration with the ICB to design and deliver the right services at the right time * Active participation within a PCN within the Eastern Locality - and as such a stakeholder via PCN representation at the Eastern Collaborative Board * The Contractor will implement Integrated Neighbourhood Teams (INTs) which should evolve from the PCN and Devon ICS. * The Contractor will explore ways of working towards becoming a provider of first resort when it comes to other struggling practices in the rest of the system. * It works with NHS Devon and its partners to help deliver the NHS Devon Strategic Framework for Primary Care (General Practice). * And demonstrate, within timeframes, how the practice will enable the service delivered to be appropriate for the list size through the period of the contract. * It will use Population Health Management data to proactively manage high risk patients and undertake regular (weekly) MDT meetings with Integrated Neighbourhood Teams or as the Devon model of Neighbourhood working requires. It is expected the Contractor will sign up to the One Devon Dataset: Population Health Management - One Devon * Ensure accurate registration with Care Quality Commission and overall rating of good or outstanding across all domains.
