This service relates to the provision of consultant led, community stroke rehabilitation, and forms part of the stroke pathway. NHS Surrey Heartlands ICB commissions 6 inpatient rehabilitation stroke beds for adults (18 years) registered with an East Surrey GP at Queen Elizabeth Foundation Neuro Rehabilitation Services (QEF). The contract has been awarded to QEF, the existing provider, following Direct Award Process C. The approximate lifetime value of the contract will be PS1.6m over the 2-year term, from 1st April 2024 until 31st March 2026.

Quality and innovation 20% * QEF has consistently maintained a service of the highest quality for stroke survivors, meeting the ICBs requirement for quality and safety. * Patients receive a structured, personalised and seamless service that enables them to feel supported through their rehabilitation. * This provider continually fulfils all quality requirements. * This provider is an integral member of the Local Stroke Working Group. * Provider delivers the Stroke Inpatient Rehabilitation specification Value 20% * The provider ensures an ability to utilise funding to maximise benefits to stroke survivors. * The service ensures individualised needs and preferences are met where it will benefit stroke survivors most, while also offering excellent value for money and delivering on outcomes. * A tailored service adds value to individuals and the system, providing a crucial prevention aspect of care to support whole system efficiency around the use of resources. * Funds will continue to be utilised efficiently and effectively to deliver a sustainable, efficient, and cost-effective service. Improving access, reducing health inequalities and facilitating choice 20% * The provider demonstrates all provision of service is accessible to all patients who are eligible for the service. * The service is available to all patients registered to an East Surrey GP who have experienced a stroke and whose rehabilitation meets the criteria for admission. Referral and admission to QEF is made in consultation with patients and family/carers ensuring all rehabilitation options in the community are explored to ensure the right rehabilitation outcome. * QEF work with acute hospitals to ensure a timely referral and admission from stroke units to QEF rehabilitation. * QEF work closely with the community Early Supported Discharge team to enable timely discharge and step down from inpatient rehabilitation to meet onward rehabilitation needs. Social value 10% * QEF is an active organisation in the local community through its charity status * The organisation provides local recruitment and training opportunities.
