Notice Title

Adults' Outreach Service

Notice Description

The Adults' Outreach Service is commissioned to enable people with assessed needs to lead a full life, maximising independence, promoting health and wellbeing and supporting people to remain safe and connected to local communities. The service is designed to support needs, and those with more complex needs, in an innovative way. The Provider will work in coproduction with the Individual and their family/ Carers (where consent is given), to support the Individual to be as independent as possible, and enabled to live a "good life" and play an active part in their community.
