Discharge to Assess and Admission Avoidance Bed-based Service.

Notice Description

The Royal Borough of Greenwich are intending to commission a single provider Contract for the provision of Adult Social Care Discharge to Assess and Step Up and Step Down Services. Potential providers are encouraged to participate in a market engagement event. The procurement process is intended to be run in accordance with the Procurement Act 2023, Light Touch Regime (LTR) above-threshold. The Service will operate from Eltham Community Hospital and will provide up to 20 beds, flexibly allocated across both pathways to support hospital discharge, admission avoidance, and short-term recovery and non-intensive rehabilitation to enable recovery and regain independence. Multidisciplinary working is key to the model, and the provider will be expected to work in partnership with system stakeholders. The service will operate on a block purchase basis and will include personal care, including nursing and medication support administered by registered nurses and care professionals care with the goal to enable service users to regain or achieve maximum independence and quality of life as soon as possible following a period of ill health. We are looking for providers with experience, skills and capability to deliver a strength-based enabling service, able to maximise people's recovery and independence during the short stay in the unit. The duration of the contract is anticipated to be 5 years, with the potential to extend for up to a maximum period/s of 2 further years

The Royal Borough of Greenwich is currently in the process of recommissioning the Discharge to Assess and Step up/Step down Service. We are currently in the process of developing our proposal for the service and we are seeking input and feedback from providers who are interested in delivering the service. As part of the procurement exercise, we are running two market engagement events. The first event was held on Tuesday, 9 December 2025 -2pm to 4pm via Microsoft Teams - thank you to the providers who were able to join in the event. This event included broad information on the proposed service model and outcomes. The second market engagement is scheduled for Wednesday 7th January 2026, from 11am-1pm through MSTeams. The event will include further information on the new care and support service model, the broad Procurement approach and Timetable, Property and lease arrangements. Potential providers will be asked for their views on the proposed plans for this service. The goal is to explore innovative and holistic approaches to improving outcomes for our residents and the health and social care system through this pathway. Your email should include names, email addresses, and job titles of those who wish to attend. Please note there is a limit of 3 attendees per organisation.
