Conwy County Borough Council seeks to appoint a suitable Service Provider to provide home adaptations for eligible residents across the county, to support individuals with chronic conditions or physical disabilities to maintain independence and improve safety and wellbeing within their homes. The contract will be applicable for a period of three years with an option to extend for a further two years.
