This specification is intended for inclusion within an Open Framework agreement designed to deliver comprehensive flexible, adaptive and transformative care and support services to individuals residing in the borough. The Open Framework will be organised into separate lots, facilitating the commissioning of both residential and nursing care provision, with the overarching aim of enabling people to age and live well in their community.

Residential Care
Standard residential care within a care home setting. It is designed to support individuals who require assistance with daily living activities, such as personal care, meals, and medication, but do not have significant nursing needs or nursing needs that cannot be met by community District Nurses
Renewal: 24 Months
Residential Dementia CareResidential care for people with a formal diagnosis of dementia, where dementia is the primary care need. Services are delivered by staff trained in dementia care, ensuring a supportive environment that minimises distress and maximises quality of life. The care home will offer adapted environments, meaningful activities, and support for families, with regular reviews to adapt care as the condition progresses. The service should include appropriate equipment to support mobility, safety, and independence, ensuring it is well maintained and suited to individual needs
Renewal: 24 Months
General Nursing CareStandard nursing care supports individuals with more complex health needs that require 24-hour nursing oversight, such as those with chronic illnesses, mobility issues, or requiring regular medical interventions. Registered nurses are always on site to provide clinical care, manage medications, and work with multidisciplinary teams to ensure comprehensive support

Nursing Dementia CareNursing dementia care is dedicated to providing comprehensive care for individuals with a primary diagnosis of dementia who also require nursing intervention. The service integrates specialist dementia care with round-the-clock registered nursing support, designed to meet the complex and evolving needs of residents whose conditions are further complicated by chronic or acute physical health challenges
Renewal: 24 Months
Short-Term ProvisionProvision of Respite Care, Short-Term Placements and Discharge to Assess (D2A) services. These services are designed to support individuals during periods of transition, enabling timely hospital discharge and promoting recovery and independence. Providers must adopt a reablement approach that empowers individuals to regain confidence in everyday skills, promotes independence, minimises reliance on long-term care, and delivers improved health and wellbeing outcomes. The aim is to deliver safe, person-centered , and flexible care that prioritises wellbeing, continuity, and dignity at every stage of support.
Renewal: 24 Months
InnovationThis lot actively promotes collaborative working with the Council to co-design, trial, and evaluate pioneering solutions in the delivery of 24-hour care. Providers are encouraged to propose and develop innovative models that address unmet needs or persistent challenges within the current system. Potential areas of innovation may include, but are not limited to: * Integration of Technology Enabled Care (TEC): For example, remote monitoring devices, telehealth consultations, digital medication management systems, and assistive technologies to enhance safety, independence, and real-time communication between care teams, individuals, and families. * Novel Service Delivery Models: Initiatives such as flexible staffing arrangements, mobile multidisciplinary teams, or community-based hubs that provide rapid outreach and wraparound support, helping to prevent unnecessary hospital admissions or support timely discharge. * Targeted Interventions for Specific Challenges: Projects focused on reducing social isolation, supporting people with complex needs, or improving transitions between care settings. This may involve peer support programmes, befriending schemes, or collaborative partnerships with voluntary and community organisations. * Use of Data and Analytics: Leveraging data to identify trends, predict risk, and tailor interventions, thereby improving outcomes and resource allocation
