Notice Description

UK4 - Tender Notice The purpose of the Service is to deliver a countywide, community embedded Social Prescribing and Community Wellness model for residents aged 25 and over living in Wrexham County Borough. In line with the National Framework for Social Prescribing, the Service provides a targeted, personalised, and non clinical wellbeing intervention for individuals with identified holistic wellbeing needs. It goes beyond general first point of contact or signposting activity by offering structured What Matters conversations, co produced personalised plans, and strength based, therapeutic style support to improve mental, physical, emotional, and social wellbeing. The Service will offer proactive, therapeutic style, non clinical interventions that go beyond simple information sharing. Practitioners will work collaboratively with individuals to understand what matters to them, co develop personalised plans, and provide hands on support to help people overcome barriers, strengthen resilience, and achieve meaningful wellbeing outcomes.

Market engagement workshop, the aim of this event is to allow potential and existing providers to discuss the future of the Provision of Social Prescribing and Community Wellness within Wrexham County Borough and ask questions about the process for the forthcoming tender opportunity. The intention is for you to meet the commissioning officer and the procurment officer involved in the Social Prescribing and Community Wellness tender process, and ask questions and give feedback about the Service and tender process. It is important that the needs of the people of Wrexham County Borough are met through the service provision, and that we can achieve good value for the money which we have to spend. The event will be held via Teams between 09:30am – 11:00am on Monday 9th March 2026.
