Notice Description

The Community Support and Prevention Service aims to provide a free at point of delivery, comprehensive, short term (up to 4 weeks), low level provision to individuals in their own homes supporting with practical tasks like preparing meals, scribe assistance for light paperwork or shopping, as well as offering companionship, helping to build independence and reducing loneliness and isolation. Referrals to the service may be from health, social care professionals or other agencies who identify a benefit or while they recover from illness, injury, or after a stay in hospital. This provision will offer individuals and their carers, choice, support improvements in quality of life, and act to deliver specific and appropriate interventions. It will provide support (and to any carers as appropriate) through the time of difficulty, enabling people to remain independent and preventing the worsening of the presenting issue. In this way, unnecessary referrals to formal emergency and crisis support services or commissioned health and care services will also be avoided.
