Hywel Dda University Health Board is seeking to renew the contract for Hospice at Home Services in Pembrokeshire Hospice at home services aim to enable patients with advanced illness to be cared for at home, and to die at home if that is their preference. Care may be provided to prevent admission to, or facilitate discharge from, inpatient care for crisis management or for longer periods of care. Care may also support times of rapid change. The Hospice at Home service is a day and night service provision. The key components of the Hospice at Home service, which are provided by an organisation registered with Care Inspectorate Wales (CIW) who have appropriate and demonstratable experience in providing Palliative and End of Life Care in people's homes are: Provide personal care and support to the patient; Provide care in a timely manner from receipt of a referral; Respite and support for carers and families.

There is a collective ambition to deliver equitable, person-centred, and high-quality care to individuals and families navigating life-limiting conditions in Wales, as reflected in the National Service Specification for Palliative and End of Life Care (PEoLC). Rooted in the values of compassion, dignity, and respect, the specification outlines a consistent standard of care across all regions and care settings in Wales. As the population ages and the prevalence of chronic and complex conditions rises, the demand for palliative care grows. Hospice at home services aim to enable patients with advanced illness to be cared for at home, and to die at home if that is their preference. Care may be provided to prevent admission to, or facilitate discharge from, inpatient care for crisis management or for longer periods of care. Care may also support times of rapid change. The Hospice at Home service is a day and night service provision. The key components of the Hospice at Home service, which are provided by an organisation registered with Care Inspectorate Wales (CIW) who have appropriate and demonstratable experience in providing Palliative and End of Life Care in people's homes are: Provide personal care and support to the patient; Provide care in a timely manner from receipt of a referral; Respite and support for carers and families. The current provider, has been providing this service in since before the transition from Pembrokeshire Local Health Board to Hywel Dda University Health Board in 2009 and has established working relationships with the Health Board's Palliative Care Team, District Nursing, GP practices and other local third sector organisations within Pembrokeshire. They are the only known provider in the Pembrokeshire area providing an established Hospice at Home service. The Health Board is awarding this contract to the existing provider following the Direct Award Process 2 for a two (2) year term, from 1st April 2026

For the avoidance of doubt, the provisions of the Public Contracts Regulations 2015/Procurement Act 2023 do not apply to this award. The publication of this notice marks the start of the standstill period. Representations by providers must be made to the relevant authority by 27th March 2026. This contract has not yet formally been awarded; this notice serves as an intention to award under the PSR Wales'.
