The service runs from Monday to Friday with flexibility to support over weekends when there are Hospital pressures. The service is responsive and works effectively with a range of partners to facilitate discharges. The successful service provider will have skills in navigating and coordinating solutions that prevent an individual being discharged home. Tasks are likely to include: * Presence within each of the hospitals in Wiltshire (GWH, RUH and SFT) and the community hospitals * Meeting with individuals on wards to understand barriers to their return home and being innovative in co-producing resolutions * Supporting individuals to pay for services or goods to support their discharge home (food, locksmiths, deep clean services) * Signposting to advice and support around benefits and accessing emergency funding * Linking up with the individual with community support available and other services to resolve complex but practical issues (befriending, shopping, meal services, accessing property etc) * Home preparation before discharge, accessing property (with consent), removal of clutter, coordinating access for equipment deliveries and ensuring utilities are accessible (gas, electric and food) * Supporting individuals with building resilience upon their return home to remain independent longer term
