The Provision of NHS high risk communities targeted lifestyle behaviour service in North Croydon on behalf of NHS South West London Integrated Care Board DISCLAIMERS Organisations considering whether to respond to this information request should note the following: * NHS South West London Integrated Care Board (the ICB) is still finalising its decision in regard to future service provision and is inviting pre-market engagement to seek the views and opinions of those organisations that may be interested in becoming involved in delivering this future service. * This Pre-Market Engagement Questionnaire (PMEQ), and any subsequent information provided in response to it, does not form an integral part of any potential future procurement exercise and should be considered as an attempt by the ICB to engage with the potential market for delivering future services; * This PMEQ, the accompanying information and the responses received arising from it are in no way legally binding on any party; * Participation in the engagement exercise is not a mandatory requirement for participating in any potential future procurement; however, responses received will assist in informing the ICB as to the level of interest from the market and will be used to evidence a decision as to whether or not to undertake a competitive procurement. SERVICE SUMMARY NHS South West London Integrated Care Board (SWL ICB) is undertaking pre-market engagement to inform the potential commissioning of a NHS High-Risk Communities Targeted Lifestyle Behaviour Service for North Croydon. The proposed service will deliver targeted, evidence-based interventions focused on weight management, smoking cessation and alcohol abstinence for residents at highest risk of preventable long-term conditions and premature mortality. The service will focus specifically on high-risk populations living in North Croydon, including individuals living in areas of socioeconomic deprivation, people from ethnic groups with elevated cardiometabolic risk at lower BMI thresholds, and individuals with severe mental illness or existing cardiovascular risk factors. The intention is to complement the existing universal prevention offer commissioned by Croydon Council by providing a targeted NHS-funded programme aimed at populations with the greatest unmet need and potential for measurable health impact. The service will deliver structured behavioural interventions, targeted outreach, and coordinated referral pathways with primary care, voluntary sector partners, and existing public health services to ensure residents can access the most appropriate support and that duplication of services is avoided. The programme will contribute to improving cardiometabolic health outcomes, reducing smoking prevalence and alcohol-related harm, and addressing health inequalities in line with NHS Long Term Plan prevention priorities and the Core20PLUS5 approach. The total indicative financial envelope for this service is PS184,539 per annum. Further detail regarding the proposed population cohorts, service scope, delivery expectations, and outcome measures is provided in the Draft Service Specification attached as Appendix A. System Alignment and Strategic Context Croydon operates within a collaborative health and care system, with both NHS and Local Authority partners contributing to the prevention and health improvement agenda. The London Borough of Croydon Public Health team is progressing plans to procure a borough-wide, universal Adult Healthy Lifestyle Service. This service is intended to provide broad access to weight management, smoking cessation and alcohol reduction for Croydon adult residents. In parallel, the NHS-commissioned High-Risk Communities Targeted Lifestyle Behaviour Service for North Croydon is being designed as a targeted intervention, focused on residents at higher risk of poor health outcomes, in line with Core20PLUS5 principles and local population health intelligence. These two offers are intended to be complementary and mutually reinforcing, rather than duplicative. The expectation is that providers will: * Understand the distinction between universal and targeted provision * Actively collaborate with Public Health-commissioned services * Establish clear referral, signposting, and escalation pathways between services * Contribute to a coherent, system-wide prevention offer that maximises reach, engagement, and outcomes Providers should therefore consider how their delivery model will align with, and add value to, the wider Croydon prevention landscape, including working in partnership to ensure residents receive the most appropriate level of support based on need.
