This procurement relates to the provision of a Lymphedema and Complex Wound Care Service to the population of Barking & Dagenham, Havering and Redbridge. The service will include, but is not limited to: Comprehensive assessment and diagnosis of lymphedema Development and implementation of individualised care plans Delivery of specialist lymphedema treatments Complex wound care assessment and management Ongoing monitoring and review of patients Patient education, prevention strategies and self-management support. Multidisciplinary working with other healthcare professionals Services will be delivered in community settings by suitably qualified and trained clinical staff, including registered nurses and HCAs. The service aims to improve patient outcomes, enhance quality of life, reduce complications and support patients to manage their conditions effectively. This is a direct award; no competition.
