Service Summary: The proposed contract will fund 1FTE Community Wellbeing Practitioner role, delivered by a provider to work in close partnership with Turning Point. Purpose: To provide support to clients in treatment with Turning Point (local treatment provider) who are presenting with low to moderate mental health needs, and without support would disengage from the service and be classed as an unsuccessful completion in treatment. Aim: To support Oldham residents with co-occurring mental health needs that are assessed to be of a low to moderate level health concern and are also accessing treatment for a substance use disorder or/and in recovery (drugs or alcohol). Outcomes and Key Deliverables: To provide robust, safe treatment for people with co-occurring conditions through multi-disciplinary working with specialist services. Our objectives are as follows: To improve support and access to mental health services for those with co-occurring conditions. To enhance engagement and trust with both services. For both services to work within a 'side-by-side' approach, providing wrap-around care with increased communication between services. To build strong working relationships between mental health and drug/alcohol misuse services. The improved wellbeing of those with co-occurring conditions. This will be measured using the nationally recognised mental wellbeing scale 'The Short Warwick-Edinburgh Mental Wellbeing Scale' (S-WEMWBS). This was chosen as it is an accessible, simple questionnaire that will be easily understood by this cohort.
