Notice Description
The provision of high-quality, self-contained accommodation with flexible, on-site care and support designed to help people live independently for longer, with improved physical, mental and social wellbeing. The service offers people aged 55 and over, their own home and care and support, depending on their individual eligible need in line with the agreed care and support plan. In exceptional circumstances the schemes may accept applications from younger adults with disabilities, offering accessible accommodation in a safe environment and delayed escalation of care needs with flexible on-site care and support to reduce or delay progression into higher cost services such as residential or nursing care. The scheme is well utilised and allows people to continue to live independently and promotes a strength-based approach when providing care and support preventing the need for permanent residential placements. It also helps individuals retain a connection to their local community as social interactions both internally and externally are encouraged.
