Notice Description

Pilot model built on the success of the homeless engagement nurse that is currently funded via the DATRIG Housing Support Grant. This funding will enable that nurse to be increased to a band 7 in order to supervise the recruitment of 1 care coordinator, and 2 peer support workers with additional GP resource. The GP resource will need to be based within Inclusion as they are the sole LLR provider for homelessness primary care. The pilot will predominantly be working with homeless substance users who have complex health care needs and are generally higher attendees of A&E. These are considered a high priority group for Leicester.
