Notice Description

This agreement relates to the utilisation of 6 self-contained apartments for short term enablement services from hospital. The apartments are to be made available by the housing supplier to support with hospital discharge processes, where there are both care requirements and accommodation requirements. There will be exclusive use of these apartments for the duration of this contract at the Stirchley House Extra Care Scheme. The apartments will be provided unfurnished. Void/Empty rooms will be managed by the council. Individuals placed in these rooms will have access to facilities on site. Registered Landlord The registered landlord will provide a scheme information pack for all Residents utilising the Enablement Service; The registered landlord will instruct the Housing Manager to co-ordinate all invoicing to the Council in relation to Rent and Service Charges for the Apartments; The registered landlord has employed the Housing Manager to be responsible for managing all housing management services at the Premises. Housing Management Provider The Housing Manager will be responsible for the collection of Rent and Service Charge for all Apartments in the Premises; The Housing Manager will arrange meter readings for all Apartments at the commencement of the utilisation of the Enablement Services; The Housing Manager will ensure that each of the Apartment's rooms are fitted out in line with the agreed fittings and fixtures as provided by the Council; The Housing Manager will ensure that the Apartments are maintained and meets all regulatory compliance requirements; The Housing Manager will ensure that all Residents receive an orientation of the Premises and provide a 'meet and greet' for new Residents; The Housing Manager will ensure that all provided facilities are well maintained and will be responsible for any repairs or maintenance at the Premises; The Housing Manager will manage day to day services to support with effective Enablement Services at the Premises.
