Summary of the contracting process

Betsi Cadwaladr University Health Board sought additional echocardiography diagnostic capacity at its Ysbyty Glan Clwyd hospital site in Bodelwyddan. The service covers performing and reporting echocardiography examinations to support the Board’s cardiac physiology provision across North Wales. It was intended to address a mismatch between demand and existing capacity, including the effects of long-term sickness and staff vacancies, and to support delivery of the required diagnostic waiting-time target. This is a clinical medical practice service. The requirement relates to the Board’s hospital-based diagnostic activity, with the wider cardiac service operating mainly Monday to Friday and including some weekend work. The selected provider is Elective Services Ltd, a large supplier based in London.

The procurement has reached the award stage: Elective Services Ltd was selected and the contract was signed on 7 September 2026. The contract value is £128,438. Betsi Cadwaladr University Health Board used a limited direct-award procedure without a prior competition, through the Shared Business Services insourcing clinical services arrangement, under the Provider Selection Regime for Wales. One provider submitted a bid, which was electronic and from outside the EU. Evaluation considered quality at 40%, value at 25%, social responsibility at 15%, collaboration and service sustainability at 10%, and improving access and reducing health inequalities at 10%. Basic selection covered activity, financial standing and technical ability. The requirement was structured as one lot.

This procurement shows demand for externally delivered echocardiography capacity within a Welsh NHS health board when internal cardiac diagnostic capacity is insufficient. A credible future competitor would need clinicians suitably qualified and experienced to perform and report echocardiography, with appropriate professional accreditation and the ability to comply with British Society of Echocardiography standards and recommendations. Providers would need to supply dependable hospital-based clinical capacity, integrate with existing cardiac physiology departments and support routine weekday delivery, with some weekend work where required. The evaluation also favoured demonstrable clinical quality, value for money, collaboration, service sustainability, social responsibility, and practical contribution to improving access and reducing health inequalities. Relevant suppliers are therefore established clinical insourcing providers able to mobilise qualified teams and maintain consistent diagnostic reporting.

Lot Information

Lot 1

Betsi Cadwaladr University Health Board (BCUHB) seeks to direct award to 'Elective Services' for the provision of echocardiography diagnostic services from the BCUHB hospital sites at Ysbyty Glan Clwyd in Bodelwyddan. BCUHB currently has three cardiac physiology departments across North Wales that provide various cardiac diagnostics which includes basic heart monitoring to implants. The service runs Monday to Friday with some weekend work. However, due to capacity and demand mismatch, long-term sickness and vacancies, there is insufficient capacity to meet the required 8-week diagnostic waiting time target. Current waiting times are excessive, and consequently, additional service capacity is required for a limited time to help reduce waiting times and support service delivery of the service. BCUHB require provision for performing and reporting echocardiography diagnostics. All clinicians are required to be suitably qualified with appropriate experience and accreditation to carry out the procedures listed. Services must comply with the standards and recommendations issued by the British Society of Echocardiography (BSE).
