Description: UK4 - Tender Notice The purpose of the Service is to deliver a countywide, community embedded Social Prescribing and Community Wellness model for residents aged 25 and over living in Wrexham County Borough. In line with the National Framework for Social Prescribing, the Service provides a targeted, personalised, and non clinical wellbeing intervention for individuals with identified holistic wellbeing needs. It goes beyond general first point of contact or signposting activity by offering structured What Matters conversations, co produced personalised plans, and strength based, therapeutic style support to improve mental, physical, emotional, and social wellbeing.
