Summary of the contracting process

NHS Surrey and Sussex Integrated Care Board commissioned a homeless and rough sleeper health service covering Eastbourne, Lewes and Wealden in East Sussex. It combines a GP clinic of up to eight hours a week with community nursing and outreach. The service is intended for people who are homeless, sleeping rough or living in temporary accommodation, helping them access mainstream healthcare and coordinating support. Work includes medical interventions, case management and care coordination, medical and risk management, proactive engagement and personalised care. The provider is expected to work flexibly with health, care, housing and support services, and to review its delivery model as service users’ needs evolve. The aim is to improve access to primary care and address health inequalities.

The contract has been awarded to Victoria Medical Centre through a direct award under the Health Care Services (Provider Selection Regime) Regulations 2023. The award date was 11 August 2026, and the service period runs from 23 November 2026 to 22 November 2028. The contract value is £471,650 over two years, with an annual value of £235,825. The process used one lot. Evaluation placed greatest weight on integration, collaboration and service sustainability (30%), followed by improving access, reducing health inequalities and facilitating choice (25%), quality and innovation (20%), value (15%) and social value (10%). The decision also considered whether the existing provider was meeting the original contract and could continue to meet the proposed requirements.

For suppliers considering similar work for this buyer, credibility rests on delivering accessible primary care and nursing for people experiencing homelessness, rough sleeping or temporary accommodation. Relevant capability includes outreach, proactive engagement, personalised care, clinical and risk management, case management and coordinated support. The service depends on effective working relationships across primary care, community nursing, housing, rough-sleeper initiatives, voluntary organisations, mental health and acute services, and local authorities; maintaining a sustainable workforce and continuity of care is therefore central. Providers need to demonstrate safe, trauma-informed, high-quality care for people with complex physical and mental health needs, alongside practical ways to reduce barriers to access and health inequalities. Effective use of a fixed financial envelope matters, while patient outcomes and integration carry greater emphasis than cost savings.
