Summary of the contracting process

Berkshire Healthcare NHS Foundation Trust is buying a proactive frailty care service for patients in Berkshire West. The provider will identify and prioritise an agreed cohort, mainly people in population-needs groups 10–11 and selected group 9 patients where clinically appropriate, using population health data, clinical judgement, frailty indicators and local knowledge. Work includes proportionate holistic frailty assessments, care-plan reviews and updates, referrals into specialist or multidisciplinary pathways, and regular multidisciplinary team participation. The service is intended to help keep patients out of acute hospital and improve coordination across the neighbourhood model. It must work with primary care, community services, social care, mental health, therapy and specialist services, and continuously review high-risk patients. Delivery is in Berkshire West. This is a health service (CPV 85100000).

The contract is recorded as awarded to The Primary Care Alliance CIC, and the contract is active. It was signed on 19 October 2026. The contract value is £558,200, with service delivery planned from November 2026 to 30 September 2027. The process used the Provider Selection Regime’s most suitable provider approach, without a published call for competition; it was not conducted under the Public Contracts Regulations 2015 or Procurement Act 2023. One bid was recorded. The evaluation gave 30% to quality and innovation, 20% to value, 15% to integration, collaboration and service sustainability, 10% to improving access, reducing health inequalities and choice, 10% to social value, and 15% to basic selection criteria. The stated award assessment was based on the key criteria, with cost weighted at zero.

This award shows demand from Berkshire Healthcare NHS Foundation Trust for neighbourhood-based, proactive frailty support that can coordinate care and reduce reliance on acute hospitals. A credible supplier would need to identify and manage high-risk cohorts, use population health data alongside clinical judgement, and provide structured frailty assessment, including mini-CGA or an equivalent where appropriate. Relevant strengths include care planning, crisis and advance-care planning, ReSPECT processes, and the ability to escalate complex needs into community, specialist and multidisciplinary pathways. Providers must be able to participate in shared decision-making and regular multidisciplinary meetings, and work effectively across primary care, community health, social care, mental health, therapy and specialist services. The evaluation placed substantial emphasis on quality, innovation, integration, collaboration, sustainability, access, health inequalities and social value, alongside basic selection standards.
