Summary of the contracting process

Guy’s and St Thomas’ NHS Foundation Trust is exploring a proactive care service for patients in mature neighbourhoods. The service would identify people whose health is becoming riskier and provide timely support, with the aims of improving outcomes, helping people maintain independent living and preventing avoidable deterioration. This is a health services requirement, with delivery locations in London. The Trust is using market engagement to understand the range of service delivery approaches available before deciding whether to proceed with a procurement. Providers with experience designing or delivering proactive, preventative care for people at rising health risk may find the service area relevant.

The process is at planning and is currently a market engagement exercise, not an invitation to tender. The Trust is seeking to understand available service delivery approaches; it anticipates publishing a further notice on 1 December 2026. The engagement milestone is scheduled for 30 November 2026. The only lot is at planning stage. The contract start and end dates shown are 1 and 2 April 2027, with a maximum extent date of 3 April 2027, but these dates should be treated as indicative: the Trust says contract duration and value remain to be determined and will be informed by engagement if it decides to procure. No procurement procedure, evaluation basis or eligibility requirements are settled at this stage.

The service is intended to intervene earlier for patients at rising risk, support independent living and reduce avoidable deterioration. Suppliers well placed to contribute are those able to offer credible models for identifying changing patient risk and arranging timely, appropriate support. The Trust is specifically seeking insight into different ways of delivering such a service, so providers with practical experience across proactive care, community health or coordinated support may be able to inform its future approach. This engagement is a chance for relevant providers to explain delivery models and capabilities before the Trust decides whether to run a procurement. No particular accreditation, staffing profile, minimum capacity or prior experience threshold has been set out so far.
