Phase 3 - COMMISSIONING OF ADDITIONAL CARE HOME PLACEMENTS TO SUPPORT PATIENTS FLOW
Notice Description
The Health Board wishes to commission beds from within the independent care home sector in Swansea Bay to support patient flow. This is in addition to normal capacity secured to meet the needs of citizens who require long term care that is commissioned by health and social care. The scheme will essentially provide ongoing NHS care for a defined period to allow patients to be discharged from hospital care pending finalisation of their ongoing care needs.The Health Board intends to commission through a combination of spot purchasing arrangements and block contracts across Swansea Bay Health Board although the precise number of beds available in each locality will depend on the response received from providers and the needs of individual patients.
